Please use the form below to submit a referral to The Advocacy Project for a Personalised Care Review meeting.

This will be a joint meeting between

• a representative from the CAMHS team;
• the child/young person
• a member of their family or their carer; and
• a PHB advisor.

The purpose of the meeting is to identify the CYP’s wellbeing goals and what is needed to support those goals. A Personal Health Budget (PHB) is an option available to them.
